Speaker: Mr Eric Lim is an independent consultant providing services in relation to Failure Analysis and Accident Investigation. He has produced more than 1200 technical investigation reports. He is also a certified instructor of Apollo Root Cause Analysis, a structured methodology to help people solving event based problems.

CPD: Certificate will be issued to IOSH members upon completion of the seminar.
Content: The talk will focus on how individuals could enhance their abilities to solve health and safety related problem by using a structured problem solving methodology. The speaker will share with the participants the reasons why many investigations failed to effectively solve a problem, and to discuss the logical thinking process that will help individuals to explore the underlying causes of incidents/accidents. Some case studies will be given.
Remarks: No attendance certificate will be issued to those who arrived after 7:15pm.
