Vegetable processing, crushed hand

A cleaner was working on the underside of a conveyor belt on a carrot processing line when his hand was
crushed. Access to dangerous parts of machinery had not been prevented. The employer had not provided sufficient and suitable information, instruction,
supervision and training.
